Case Manager

Part Time 32 hours per week 

Outpatient Post Acute Brain Injury 

Shepherd Center's Day Program enables patients with brain injury who no longer need 24-hour nursing care to continue their recovery and rehabilitation.

Shepherd Center's Day Program takes place at Shepherd Pathways, a nearby facility in Decatur, Ga., and includes treatment rooms, two large therapy gyms, a computer lab, outdoor sports equipment and putting green for recreational and leisure activities.


Case Management is a collaborative process performing the primary role functions of assessment, planning, facilitation and advocacy for options and services to meet an individualís health needs. Responsibilities include utilization management, discharge planning and discharge and wellness management. Case Managers lead the team toward optimal client outcomes all of which include value, quality, reliability, safety and cost efficiency.

Education: Degree in nursing, social work, physical therapy, speech pathology, occupational therapy, or
recreation therapy preferred. Clinical degree in Allied Health Professional required, Masterís degree preferred,
or Eligible to sit for the Certification in Case Management (CCM or CMC),CRRN, CRC, CDMS.
Experience: Experience in healthcare environment. Two years preferred
Licensure: Nursing, SW, CCM, CMC, CRRN, CRC, CDMS preferred
Certification: Eligible to sit for the CCM or CMC. CRRN preferred for nurses
